Fields validated
- 25.14 — Service Frequency Code
- 25.15 — Service Duration
Duration without frequency is incomplete
A duration such as 0030 means “30 minutes per session,” but it does not say whether the service schedule is expressed using the applicable day, week, month, or year frequency from the current CALPADS code set. Both values must reflect the same service statement in the operative plan.
| Evidence in the operative plan | Correct treatment | Avoid |
|---|---|---|
| Frequency and minutes are documented | Populate 25.14 with the current supported code and retain the accurate 25.15 duration. | Guessing the frequency from past records. |
| Duration was populated by mistake | Clear 25.15 if the record should not report that value and applicable requirements permit it. | Adding an arbitrary frequency solely to clear the error. |
| Source schedule is ambiguous | Return the record to special education program staff for clarification. | Reverse-engineering the plan from billing or attendance totals. |
Correct the pair, not merely the blank. Frequency and duration together must accurately express the planned service schedule.
Recommended correction path
- 1
Identify the controlling plan
Match the SERV row to the correct student, service, provider, and Plan Effective Start Date.
- 2
Read the complete service statement
Establish both the session minutes and the documented frequency.
- 3
Select the current frequency code
Use the CALPADS v18 code set and preserve any leading zero required by the two-character field.
- 4
Correct the source mapping
Ensure the special education system exports frequency whenever it exports Service Duration.
- 5
Resubmit and confirm
Verify SERV0577 clears without producing GERR0002, SERV0329, SERV0330, or FRMT0004.
