Recognize the identifier
CFS v18 specifies a nine-character structure: 1A-CountyCode-3 digits. Example: 1A-19-123. The structure alone is not enough—the complete identifier must correspond to the correct agency in the Special Education Division’s current NPA data.
| Submitted information | Question to answer | Likely action |
|---|---|---|
| Correct-looking nine-character value | Is it assigned to the actual provider and valid for the relevant period? | Confirm against the current CDE certification data. |
| NPS school CDS code | Was a school identifier substituted for an agency identifier? | Locate the actual service-providing NPA or correct the provider type. |
| Agency name or contract/vendor number | Did the export map a local identifier into 25.05? | Correct the crosswalk to the CDE NPA identifier. |
| Former or non-certified agency | Was the agency valid for the plan/service context? | Reconcile provider documentation and current reporting requirements before changing the record. |
Recommended correction path
- 1
Confirm that the provider is an NPA
Review provider code 25.12, the operative plan, and service arrangement; an NPA record should use provider code 400.
- 2
Inspect the literal identifier
Check all nine characters, hyphens, county digits, leading characters, and accidental spaces.
- 3
Validate the agency
Match the identifier and agency name against the authoritative CDE NPA resource and the relevant service documentation.
- 4
Correct the supported field
Fix an incorrect NPA identifier, or clear it and use the correct provider code if the service was not provided by an NPA.
- 5
Repair the crosswalk and verify
Update the source-system provider mapping, then confirm SERV0447, SERV0327, and SERV0328 are all resolved.
