Things to check for in submission: Verify that the Part C to B Transition Plan in IFSP is populated if the student is greater than 2 years 9 months on Census day, is eligible and participating, and on an IFSP plan on census day at the LEA where they are enrolled on Census day. Suggested Resolution: 1) Populate the Part C to B Transition Plan in IFSP indicator. 2) Verify the correct Education Plan Type Code is used. 3) Verify the most recent SWDS record effective on during the report period
