CFS v18 defines the NPA identifier as nine characters in the pattern 1A-CountyCode-3 digits, such as 1A-19-123. It is an agency identifier—not an NPS school CDS code, vendor name, contract number, or SELPA code.

Establish who actually provides the service

Use the operative IEP, IFSP, or ISP together with the local special education system and service arrangement. If an NPA is the provider, report code 400 and the certified agency’s identifier. If the LEA, SELPA, county office, another public agency, or another valid provider type actually provides the service, use the appropriate current provider code and leave the NPA identifier blank.

Do not populate an arbitrary NPA identifier merely to clear the error. First prove the provider type and the specific agency from the student’s plan and service records.

    Match SSID, service code, provider, location, and Plan Effective Start Date to the intended SERV row.

    Verify the documented provider

    Review the operative plan and service arrangement to determine whether an NPA actually provides this service.

    Validate the agency identifier

    If code 400 is correct, obtain the current nine-character identifier from the authoritative NPA record rather than copying an NPS CDS code.

    Correct the owning system

    Update the special education data system or provider mapping so code 400 and field 25.05 are generated together.

    Resubmit and verify

    Confirm SERV0327 clears and check for SERV0328 or SERV0447, which identify a conflicting provider code or invalid agency identifier.
