Current, retired, and not-yet-effective are different
| Status | Meaning | Correct response |
|---|---|---|
| Effective on the record date | The code may be used if it also describes the service accurately. | Check for wrong field placement or an incorrect Plan Effective Start Date. |
| Retired before the record date | The value once existed but is no longer accepted for that date. | Use the valid successor only if it truthfully represents the documented service. |
| Effective after the record date | The code was introduced later. | Use the code valid at the time; do not rewrite history with a newer value. |
Do not automatically replace every retired code with today’s code. Historical SERV records must use the value valid for their effective date and supported by the operative plan.
Recommended correction path
-
1
Identify code and controlling date
Capture the rejected field, value, and 25.17 Plan Effective Start Date.
- 2
Check the code’s date range
Determine whether it is retired, premature, or valid on that date.
- 3
Verify the plan history
Confirm which service terminology and effective date the approved plan actually supports.
- 4
Correct code, date, or mapping
Change only the element proven inaccurate; do not move a date merely to make a code pass.
- 5
Audit the retired mapping
Check all SERV records produced from the same local code.
