Recognized does not automatically mean appropriate. After replacing an invalid value, confirm the new code accurately represents the documented service and is effective for the relevant date.

Recommended correction path

    Capture the exact rejected value

    Preserve capitalization, leading zeroes, spaces, and punctuation.

    Confirm column placement

    A valid provider code in the service field is still invalid.

  3. 3

    Compare with the current code set

    Use the value effective for the Plan Effective Start Date; do not rely on an old crosswalk.

    Verify meaning against the plan

    Confirm service, provider, location, frequency, and duration describe the same service event.

    Correct the mapping and resubmit

    Audit other records generated from the same local value.
