The conference date records when the transition conference occurred; the indicator separately reports whether the transition plan was included in the IFSP. One does not answer the other.

Part C-to-B transition sequence

Part C / IFSPPlan Type 150 governs the early-intervention PLAN segment.
Transition planningField 23.35 reports whether the plan was included in the IFSP.
Transition conferenceField 23.34 reports the actual conference date.

    Use membership/accountability records or the child’s IFSP transition evidence rather than defaults or assumptions.

    Place organizational relationships or birth, IFSP, transition plan, conference, and Part B events in order.

    Correct the owning source

    Populate the documented indicator when the conference date is valid, or remove/correct the conference date when no such conference occurred. Repair recurring crosswalk or copied-forward logic.

    Resubmit and reconcile

    Confirm acceptance and review connected SENR, SWDS, MEET, PLAN, SERV, SELPA, and DSEA history.
