This field documents a Part C-to-B transition plan inside an IFSP. It does not belong on an IEP (100) or ISP (200), even if the student has a historical transition record.

Part C-to-B transition sequence

Part C / IFSPPlan Type 150 governs the early-intervention PLAN segment.

    Place organizational relationships or birth, IFSP, transition plan, conference, and Part B events in order.

    Correct the owning source

    Clear 23.35 from a non-IFSP PLAN segment, or correct Plan Type only when the governing plan is actually an IFSP. Repair recurring crosswalk or copied-forward logic.

    Resubmit and reconcile

    Confirm acceptance and review connected SENR, SWDS, MEET, PLAN, SERV, SELPA, and DSEA history.
