Part C to B Transition Plan in IFSP Indicator must be populated if Special Education Plan Type equals 150 (IFSP), and the student's age on the Plan Effective Start Date is equal to or greater than 2 years 9 months

    Use membership/accountability records or the child’s IFSP transition evidence rather than defaults or assumptions.

    Place organizational relationships or birth, IFSP, transition plan, conference, and Part B events in order.

    Correct the owning source

    Populate the documented Y/N indicator, or correct Birth Date, Plan Type, or Effective Date only when authoritative evidence shows it is wrong. Repair recurring crosswalk or copied-forward logic.

    Resubmit and reconcile

    Confirm acceptance and review connected SENR, SWDS, MEET, PLAN, SERV, SELPA, and DSEA history.
