In Plain Language: After collecting completed assessments, the district aggregates the required counts by school, reconciles them to the eligible roster, and submits by July 1 through a system designated by the state dental director, the county office of education, or both under current instructions.

Why this matters

The aggregate report shows whether pupils received assessments, faced cost or access barriers, had decay, or never returned documentation. Reliable categories help direct oral-health resources without exposing pupil records.

Primary ownership

Primary owner: district health-services/KOHA reporting lead. Handoff partners: school nurses/designated collectors, enrollment/data staff, privacy, superintendent designee, county office, and the current state reporting system.

Operational workflow

  1. 1. Obtain a final eligible-pupil and status file from every school after the May 31 closeout.
  2. 2. Validate mutually exclusive categories and reconcile totals by school to the pupil-level roster.
  3. 3. Calculate the eight statutory counts: subject pupils; proof received; financial burden; lack of access; nonconsent; caries experience; untreated decay; and no returned assessment/waiver.
  4. 4. Confirm the current reporting route, credentials, school identifiers, and privacy controls.
  5. 5. Submit by July 1, retain confirmation, and correct discrepancies through the authorized process.
