Provide Type 2 Diabetes Information to Parents of Incoming Seventh-Grade Pupils

In Plain Language: Give parents/guardians of incoming seventh graders the CDE-developed type 2 diabetes information. It describes the condition, risk factors and warning signs, screening tests, prevention/treatment context, and the recommendation to consult a health-care provider.

Why this matters

The notice supports awareness at an age when risk may become more visible. It must inform without stigmatizing body size, race/ethnicity, family history, disability, or health status.

Joint ownership

Notice owner: enrollment/health services. Partners: middle-school administration, counseling, physical education, nutrition, language access, communications, and Section 504/IEP staff as applicable.

Operational workflow

  1. 1. Confirm the current CDE information and translations; define the incoming seventh-grade population.
  2. 2. Distribute in an accessible, understandable format before or at entry and retain delivery evidence.
  3. 3. Provide a private route for questions and referrals to health-care/community resources.
  4. 4. Train staff to avoid stigma and to respond appropriately to concerning symptoms or a disclosed diagnosis.
  5. 5. For diagnosed pupils, separately activate medical orders, accommodations, trained coverage, meals/activity support, and emergency planning.

The notice should answer

  • What is type 2 diabetes?
  • What risk factors and warning signs may justify discussing screening with a provider?
  • What screening tests may a provider use?
  • Where can a family obtain reliable medical guidance?
