Verify and Record Immunization Compliance Before Admission or Seventh-Grade Advancement

An enrollment gate for validating acceptable records, applying current dose and exemption rules, preserving immediate-enrollment protections, and documenting the correct status.

In Plain Language: Before ordinary admission, inspect an acceptable immunization record, transfer valid dates to the California School Immunization Record (Blue Card/CDPH 286) or equivalent, and classify the pupil as complete, legitimately conditional, exempt/other, or overdue. Seventh-grade advancement requires the current pertussis/Tdap evidence.

Why this matters

A checkbox cannot establish compliance. Staff must assess dose dates, age/grade, intervals, exemptions, and whether the pupil qualifies for a narrow records-awaiting or protected-population admission rule.

Joint ownership

Clinical owner: school nurse/health services. Gate owner: enrollment/registrar. Partners: homeless/foster liaisons, special education, CAIR-ME/records staff, prior schools, and local public health.

Operational workflow

  1. 1. Confirm whether the event is admission, re-entry, transfer, or seventh-grade advancement and apply the correct current table.
  2. 2. Review an acceptable source record; validate vaccine, dose date, age/grade, interval, disease-history evidence, and CAIR-ME exemption status.
  3. 3. Record dates and classification on the Blue Card/equivalent; identify due dates and protected-population/records-awaiting rules.
  4. 4. Admit only when complete, lawfully conditional, exempt/other, or protected by a specific immediate-enrollment rule.
  5. 5. File the record, schedule follow-up, and include required checkpoint reporting.

Do not collapse these statuses

  • Complete: All applicable requirements documented.
  • Conditional: Series started and no dose currently due, or another permitted conditional category.
  • Awaiting transfer record: Up to 30 school days in qualifying transfers; separate immediate-enrollment protections apply.
  • Overdue: Requirement is due now; ordinarily not admissible until corrected.

  • Admission/advancement determination
  • Dose/date/interval and CAIR-ME verification
  • Conditional or protected-enrollment deadline
  • Decision, notice, follow-up, and reporting record
