In Plain Language: Collect hearing-testing results from every district school and included charter school, reconcile them to the PM 100 instructions and October CALPADS enrollment, and submit one combined district form. Individual school forms are no longer accepted.

Why this matters

PM 100 is not merely a count of screenings. It connects enrollment, required testing, confirmed failures, referrals, and documented medical/audiological follow-up—and exposes missing handoffs.

Joint ownership

Report owner: district health-services lead/school nurse administrator. Data partners: registered school audiometrists, charter liaisons, CALPADS/enrollment staff, schools, and district administration.

Operational workflow

  1. 1. At year start, inventory testing sites, included charter schools, registered personnel, required grades, waivers, and data owners.
  2. 2. Maintain school-level screening, threshold, referral, and follow-up records throughout the cycle.
  3. 3. Obtain October enrollment counts and reconcile each site's totals and exceptions.
  4. 4. Combine all school and charter data into one current PM 100; perform cross-foot and missing-follow-up checks.
  5. 5. Obtain approval, submit by June 30 through the current DHCS channel, and retain delivery evidence.
