In Plain Language: Confirm eligible equipment and services are delivered, installed, operational as required, accepted, inventoried, and supported by contract, invoice, payment, and location evidence by the applicable FRN deadline. If a qualifying delay requires an extension, file Form 500 by the current service-delivery deadline unless USAC already extended it automatically.

Why this matters

A purchase order or warehouse delivery may not prove completed eligible service. Missing installation, acceptance, asset location, applicant share, or timely extension can make committed funding unrecoverable.

Primary ownership

Primary owner: E-Rate applicant lead with technology project manager. Partners: service provider, procurement, receiving, facilities, network staff, fiscal/accounts payable, inventory custodian, and EPC authorized user.

Operational workflow

  1. 1. Identify all FY2026 non-recurring non-fiber FRNs and verify the current service-delivery deadline for each.
  2. 2. Reconcile contract scope, approved quantities, eligible locations, schedule, delivery, installation, testing, and acceptance.
  3. 3. Resolve substitutions, SPIN changes, reductions, or scope discrepancies before claiming.
  4. 4. If an eligible delay applies and no automatic extension appears, submit Form 500 on or before the current deadline.
  5. 5. Complete service by September 30 or the approved extended date; inventory assets and prepare invoice support.
