In Plain Language: Review the immunization status of every pupil in the required grades, complete the current CDPH worksheet/categories, submit each school’s report through the CAIR Hub Immunization Reporting application by the published deadline, and retain confirmation.

Why this matters

Annual reports measure community protection and compliance with school immunization law. An aggregate report is only as reliable as each cumulative-file record, conditional-admission follow-up, exemption documentation, and school roster.

Primary ownership

Primary owner: school nurse or immunization reporting designee. Handoff partners: registrar, school administrators, district health lead, CAIR Hub administrator, attendance, special education/alternative programs, families, and local health department.

Operational workflow

  1. 1. Confirm every reporting school, current CAIR Hub account, assigned contacts, school code, grade span, and live deadline.
  2. 2. Build TK/K and grade 7 rosters and reconcile them to cumulative records, including pupils with exemptions or special admission circumstances.
  3. 3. Use current CDPH instructions to classify each pupil; resolve missing, conditional, overdue, and conflicting documentation through the lawful follow-up process.
  4. 4. Complete and independently review each school’s aggregate worksheet/report.
  5. 5. Submit in CAIR Hub, confirm the green Completed status/email, and retain the source calculation and any later correction.
